High costs, long wait times, and insufficient access to care in rural areas are just a few of the issues that plague us \u2013 and right now, it\u2019s getting worse.\u00a0<\/span><\/p>\n<p>&nbsp;<\/p>\n<p><span style=\"font-weight: 400;\">This year, the Trump Administration has taken an ax to countless programs, grants, and funding initiatives in the healthcare realm, slashing anything seeking to make wellness a more accessible and diverse industry. By the end of August, the number of terminations had reached <\/span><a href=\"https:\/\/grant-witness.us\/nih-data.html\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">over 5,000 grants<\/span><\/a><span style=\"font-weight: 400;\"> totaling more than $4.5 billion in federal funding. The people most affected? Women, the LGBTQ+, and racial minorities.\u00a0<\/span><\/p>\n<p>&nbsp;<\/p>\n<p><span style=\"font-weight: 400;\">Research on the health and well-being of these groups is already decades behind the available knowledge on white, cisgender men and women (primarily men), who are seen as the standard for many studies. Science was finally beginning to catch up, only to get cut off at the knees.\u00a0<\/span><\/p>\n<p>&nbsp;<\/p>\n<p><span style=\"font-weight: 400;\">And it\u2019s not just official research \u2013 our current culture wars are heavily targeting LGBTQ+ people, particularly those who don\u2019t ascribe to gender norms. Increasing distrust in healthcare systems is leading many queer people to simply forgo their routine care, putting them at a higher risk of disease, chronic pain, or even early death.\u00a0<\/span><\/p>\n<p>&nbsp;<\/p>\n<p><span style=\"font-weight: 400;\">In a nutshell: healthcare isn\u2019t built for queer people.<\/span><\/p>\n<h2><strong>Healthcare: It\u2019s Not For Everyone\u00a0<\/strong><\/h2>\n<p><span style=\"font-weight: 400;\">The statistics are sobering: 61% of LGBTQ+ people report having had a <\/span><a href=\"https:\/\/www.kff.org\/racial-equity-and-health-policy\/lgbt-adults-experiences-with-discrimination-and-health-care-disparities-findings-from-the-kff-survey-of-racism-discrimination-and-health\/#:~:text=LGBT%20adults%20are%20also,prescribe%20needed%20pain%20medication\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">negative experience with a healthcare provider<\/span><\/a><span style=\"font-weight: 400;\"> in the last 3 years. Another 60% expect to face <\/span><a href=\"https:\/\/www.kff.org\/racial-equity-and-health-policy\/lgbt-adults-experiences-with-discrimination-and-health-care-disparities-findings-from-the-kff-survey-of-racism-discrimination-and-health\/#:~:text=Additionally%2C%20six%20in%20ten%20LGBT%20adults%20say%20they%20prepare%20for%20insults%20from%20health%20care%20providers%20or%20staff%20or%20feel%20that%20they%20need%20to%20be%20careful%20about%20their%20appearance%20to%20be%20treated%20fairly%20at%20least%20some%20of%20the%20time%20when%20seeking%20care%2C%20compared%20to%20four%20in%20ten%20(39%25)%20non%2DLGBT%20adults%20who%20say%20the%20same\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">insults or discrimination<\/span><\/a><span style=\"font-weight: 400;\"> at their doctor\u2019s office. Caring for nonbinary and transgender individuals requires extra education that many medical schools don\u2019t cover, leaving it up to the healthcare provider to attempt learning it on their own.\u00a0<\/span><\/p>\n<p>&nbsp;<\/p>\n<p><span style=\"font-weight: 400;\">Even a step as easy as checking in at the doctor can lead to anxiety. Intake forms and health insurance companies often force you to choose between male and female, leaving little room for those outside the binary. Providers make assumptions and judgments, so LGBTQ+ people get left out of conversations about their own health.\u00a0<\/span><\/p>\n<p>&nbsp;<\/p>\n<p><span style=\"font-weight: 400;\">If it doesn\u2019t affect you personally, it may seem like a non-issue; after all, LGBTQ+ people only make up about 9% of the general population. But consider it this way: 9% of America is around 32 million people, equaling roughly the population of Texas. That\u2019s a lot of people for healthcare to fail.\u00a0<\/span><\/p>\n<p>&nbsp;<\/p>\n<p><span style=\"font-weight: 400;\">All of these problems become compounded if the LGBTQ+ person is also Black, Hispanic, or another racial minority. But awareness isn\u2019t enough \u2013 how can we queer wellness and make it inclusive for everyone?\u00a0<\/span><\/p>\n<h2><strong>Building a Future Beyond the Binary\u00a0\u00a0<\/strong><\/h2>\n<p><span style=\"font-weight: 400;\">Federal and state governments are restricting LGBTQ+ healthcare more than ever before in American history. To date, 27 states have implemented laws barring <\/span><a href=\"https:\/\/www.kff.org\/lgbtq\/gender-affirming-care-policy-tracker\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">youth access to gender affirming care<\/span><\/a><span style=\"font-weight: 400;\">, with some proposing restrictions through age 26. Many states are attempting to prevent health insurance plans from paying for gender affirming care for people of all ages.\u00a0<\/span><\/p>\n<p>&nbsp;<\/p>\n<p><span style=\"font-weight: 400;\">As the American College of Physicians <\/span><a href=\"https:\/\/www.acponline.org\/advocacy\/state-health-policy\/attacks-on-gender-affirming-and-transgender-health-care#:~:text=On%20Inauguration%20Day,other%20policy%20directives.\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">wrote<\/span><\/a><span style=\"font-weight: 400;\"> in August: \u201cOn Inauguration Day for his second term, President Trump issued an executive order asserting that sex is \u201cnot changeable\u201d and ordering federal policy changes including withholding federal funding from all institutions that \u201cpromote gender ideology,\u201d restricting access to medical care for transgender prisoners\u2026and not allowing gender marker changes on US passports.\u201d\u00a0<\/span><\/p>\n<p>&nbsp;<\/p>\n<p><span style=\"font-weight: 400;\">But despite these legal challenges, there are still several key ways that healthcare providers can make their practices more inclusive to the LGBTQ+ community. Adopting these can mean saving the life of a queer person, and it starts with compassion.\u00a0<\/span><\/p>\n<h3><strong>Learn the Language\u00a0<\/strong><\/h3>\n<p><span style=\"font-weight: 400;\">One of the easiest and most helpful ways to build an inclusive medical practice starts with educating yourself on the language, terms, and labels used by the LGBTQ+ community. It\u2019s a respectful way to show patients that you care.\u00a0<\/span><\/p>\n<p>&nbsp;<\/p>\n<p><span style=\"font-weight: 400;\">Find an <\/span><a href=\"https:\/\/www.aecf.org\/blog\/lgbtq-definitions\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">LGBTQ+ glossary<\/span><\/a><span style=\"font-weight: 400;\"> and study it, as even small differences in identity can affect the care a person needs. Many online resources offer guides teaching healthcare workers <\/span><a href=\"https:\/\/lgbtqiahealtheducation.org\/wp-content\/uploads\/Providing-Inclusive-Services-and-Care-for-LGBT-People.pdf\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">how to talk to LGBTQ+ people<\/span><\/a><span style=\"font-weight: 400;\"> thoughtfully.\u00a0<\/span><\/p>\n<p>&nbsp;<\/p>\n<p><span style=\"font-weight: 400;\">When in doubt, ask the patient how they identify and how they would like to be addressed. Most importantly, avoid slang terms, personal biases, and judgments. Does it use inclusive language and photos? This is particularly important for online or telehealth providers, as that\u2019s a patient\u2019s main impression of your offerings.\u00a0<\/span><\/p>\n<p>&nbsp;<\/p>\n<p><span style=\"font-weight: 400;\">Next, look at your physical space, if you have one. Include resources like pamphlets on LGBTQ+ health, queer magazines, or <\/span><a href=\"https:\/\/www.ama-assn.org\/sites\/ama-assn.org\/files\/corp\/media-browser\/public\/glbt\/nondiscrimination_0.pdf\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">non-discrimination statements<\/span><\/a><span style=\"font-weight: 400;\"> welcoming people of all identities. These messages, while subtle, will help set LGBTQ+ people at ease.\u00a0<\/span><\/p>\n<p>&nbsp;<\/p>\n<p><span style=\"font-weight: 400;\">As patients check in, intake forms matter. Instead of simply using male and female checkboxes, ask for sex assigned at birth, current gender, preferred pronouns, sexual identity, and legal name vs. chosen name.\u00a0<\/span><\/p>\n<p>&nbsp;<\/p>\n<p><span style=\"font-weight: 400;\">Finally, provide staff training, if applicable. Educate them on properly using a patient\u2019s preferred terms and making them feel comfortable. Hiring queer staff is an excellent way to support the community directly and make your practice more inclusive.\u00a0<\/span><\/p>\n<h3><strong>Stay Up To Date\u00a0<\/strong><\/h3>\n<p><span style=\"font-weight: 400;\">LGBTQ+ healthcare is evolving rapidly as new forms of treatment \u2013 whether that\u2019s hormone replacement therapy, updates in gender affirming care, or transitional care needs \u2013 continue to be developed and implemented. Attending seminars or reading medical journals that address these treatments can help you stay informed.\u00a0<\/span><\/p>\n<p>&nbsp;<\/p>\n<p><span style=\"font-weight: 400;\">It\u2019s also vital to remember and acknowledge that LGBTQ+ people face different health issues than straight or cisgender people. For example, transgender men will still need pap smears and OB-GYN care, while lesbian women are more likely to suffer from polycystic ovarian syndrome. Personal beliefs toward any group have no place in a treatment setting, and they can manifest in asking derogatory questions or failing to acknowledge a patient\u2019s unique needs. This may ultimately shatter the patient\u2019s trust in healthcare providers, leading them to neglect their own wellness down the road.\u00a0<\/span><\/p>\n<h3><span style=\"font-weight: 400;\"><strong>Fight for Queer Rights<\/strong>\u00a0<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">The government is, undeniably, attacking LGBTQ+ rights, and it\u2019s on all of us to stand up for the community. Whether you\u2019re queer yourself or just an ally, everyone has the ability to demand change. Restricting healthcare for any group will impact the entire field if we don\u2019t stop it.\u00a0<\/span><\/p>\n<p>&nbsp;<\/p>\n<p><span style=\"font-weight: 400;\">Change at the personal level is an excellent way to start, but don\u2019t let that be your stopping point \u2013 join movements at the state and federal level.
