The condition, marked by <\/span><a href=\"https:\/\/my.clevelandclinic.org\/health\/diseases\/mast-cell-activation-syndrome#:~:text=Mast%20cell%20activation%20syndrome%20(MCAS)%20is%20when%20you%20have%20unexplained%20episodes%20of%20severe%20symptoms%20like%20swelling%2C%20diarrhea%2C%20vomiting%2C%20flushing%20and%20itching.\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">unexplained bouts of symptoms<\/span><\/a><span style=\"font-weight: 400;\"> like swelling, diarrhea, vomiting, flushing, and itching, scientists only gave the condition a modernized classification and diagnostic criteria when they gathered at a <\/span><a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC11212760\/#:~:text=In%202010%2C%20a%20conference%20was%20organized%20to%20elaborate%20on%20this%20topic%20to%20modernize%20the%20classification%20and%20diagnostic%20criteria%20for%20mast%20cell%20disorders%2C%20especially%20MCAS.\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">special conference in 2010<\/span><\/a><span style=\"font-weight: 400;\">.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Despite increased conversation around the condition through advocacy and internet spaces, we <\/span><a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC11212760\/#:~:text=Epidemiologic%20studies%20are%20not%20yet%20sufficient%20to%20verify%20the%20incidence%20and%20prevalence%20of%20both%20mastocytosis%20and%20MCAS.\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">don\u2019t know how many people<\/span><\/a><span style=\"font-weight: 400;\"> truly have MCAS. In fact, studies have shown that MCAS is <\/span><a href=\"https:\/\/onlinelibrary.wiley.com\/doi\/10.1111\/all.15304#:~:text=Conclusion,in%20patients%20with%20suspected%20MCAS.\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">more commonly suspected than diagnosed<\/span><\/a><span style=\"font-weight: 400;\">\u2014meaning other factors or triggers cause symptoms that are mistaken for MCAS.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">But those diagnosed with MCAS or <\/span><a href=\"https:\/\/www.veriheal.com\/blog\/medical-cannabis-histamine-intolerance-mcas\/\"><span style=\"font-weight: 400;\">histamine intolerance<\/span><\/a><span style=\"font-weight: 400;\"> know all about the uncomfortable, painful symptoms it can cause. As awareness increases, more people are wondering if cannabis can help with MCAS. Let\u2019s investigate.\u00a0<\/span><\/p>\n<h2><b>MCAS Diagnosis and Symptoms\u00a0<\/b><\/h2>\n<p><span style=\"font-weight: 400;\">The symptoms associated with MCAS occur because of an inappropriate immune response. Mast cells are <\/span><a href=\"https:\/\/my.clevelandclinic.org\/health\/diseases\/mast-cell-activation-syndrome#:~:text=Mast%20cells%20are%20a%20part,to%20flush%20out%20the%20intruder.\"><span style=\"font-weight: 400;\">part of the immune system<\/span><\/a><span style=\"font-weight: 400;\"> and help to protect the body from harmful pathogens. When they sense a trigger, they release histamine and other chemicals to activate the immune system. For people with MCAS, the <\/span><a href=\"https:\/\/my.clevelandclinic.org\/health\/diseases\/mast-cell-activation-syndrome#:~:text=Your%20mast%20cells%20reacting%20when%20they%20shouldn%E2%80%99t%20causes%20MCAS.\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">body misinterprets various triggers<\/span><\/a><span style=\"font-weight: 400;\"> and sets off an unnecessary immune response.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">MCAS is marked by repeated episodes of severe symptoms across multiple bodily systems. For MCAS diagnosis, patients must have symptoms in <\/span><a href=\"https:\/\/my.clevelandclinic.org\/health\/diseases\/mast-cell-activation-syndrome#:~:text=To%20be%20considered,stomach%20and%20intestines)\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">two or more bodily systems<\/span><\/a><span style=\"font-weight: 400;\">, including skin, respiratory, cardiovascular, and digestive systems.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u00a0The Cleveland Clinic recognizes the <\/span><span style=\"font-weight: 400;\">following MCAS symptoms<\/span><span style=\"font-weight: 400;\">:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Low blood pressure (feeling dizzy or lightheaded)<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Flushing (hot, red, or pink skin)<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Itchy skin<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Hives<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Nasal congestion<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Swelling, often of your face, lips, eyes, tongue or throat (angioedema)<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Constipation or diarrhea (sometimes alternating)<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Abdominal (belly) pain<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Vomiting<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Shortness of breath<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Weakness or fainting<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Anaphylaxis (severe allergic reaction, which may include a combination of the above symptoms)<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">To receive a diagnosis, healthcare providers will look at symptoms and evaluate how you react to MCAS treatment and tests. Doctors may also <\/span><a href=\"https:\/\/my.clevelandclinic.org\/health\/diseases\/mast-cell-activation-syndrome#:~:text=MCAS%20testing,you%20have%20MCAS.\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">request blood tests<\/span><\/a><span style=\"font-weight: 400;\"> like tryptase and immunological studies, and rule out other allergens through skin testing.\u00a0<\/span><\/p>\n<h2><b>MCAS Treatment\u00a0<\/b><\/h2>\n<p><a href=\"https:\/\/www.aaaai.org\/conditions-treatments\/related-conditions\/mcas#:~:text=The%20goals%20of,reduce%20anaphylactic%20episodes.\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">To manage an MCAS episode<\/span><\/a><span style=\"font-weight: 400;\">, experts recommend following the same protocols for anaphylaxis, starting with epinephrine, if symptoms are severe. In the long-term, antihistamines are also recommended to help with itching, abdominal discomfort, and flushing. Experts recommend second-generation antihistamines (like loratadine and cetirizine) over first-generation (like hydroxyzine and diphenhydramine) because they have fewer side effects.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">There are also more ways to help manage specific symptoms. For example, aspirin also blocks the production of prostaglandin D2, which can help reduce flushing. Additionally, drugs like montelukast and zafirlukast can help reduce wheezing and abdominal cramps.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Omalizumab may help to reduce reactivity and mast cell activation, reducing the occurrence of anaphylaxis. As a last resort for edema, hives, and wheezing, patients can use corticosteroids.\u00a0<\/span><\/p>\n<h2><b>Cannabis for MCAS: The Science\u00a0<\/b><\/h2>\n<p><span style=\"font-weight: 400;\">Given MCAS awareness has only recently increased, some people with the condition are looking for alternative ways to manage symptoms. This is especially true for people who have negative reactions to prescribed medications or are seeking additional support.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Like many conditions, there is not a lot of research to back up the use of cannabis for mast cell activation syndrome. However, this does not mean there is no potential for the two to work together.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">It\u2019s generally accepted that <\/span><a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC1138953\/#:~:text=Cannabinoids%20are%20broadly%20immunosuppressive%2C%20and,is%20expressed%20in%20mast%20cells.\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">cannabinoids are immunosuppressive and anti-inflammatory<\/span><\/a><span style=\"font-weight: 400;\">. Cannabinoid receptor 2 (CB2) is a typical constituent of immune system cells. Interestingly, researchers have also established that cannabinoid receptor 1 (CB1) is expressed in mast cells.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The recent discovery that the immune system uses both types of cannabinoid receptors opens the door to many potential new immune system-related treatments with cannabis. This could potentially include mast cell activation syndrome and <\/span><a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC11212760\/#:~:text=Common%20symptoms%20typically,one%20by%20one.\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">co-occurring symptoms<\/span><\/a><span style=\"font-weight: 400;\"> like <\/span><a href=\"https:\/\/www.veriheal.com\/conditions\/chronic-pain\/\"><span style=\"font-weight: 400;\">chronic pain<\/span><\/a><span style=\"font-weight: 400;\">, <\/span><a href=\"https:\/\/www.veriheal.com\/blog\/new-research-on-cannabis-and-anxiety-relief\/\"><span style=\"font-weight: 400;\">heightened anxiety<\/span><\/a><span style=\"font-weight: 400;\">, neurological symptoms, or <\/span><a href=\"https:\/\/www.veriheal.com\/conditions\/mood-disorders\/\"><span style=\"font-weight: 400;\">mood disorders<\/span><\/a><span style=\"font-weight: 400;\">.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">In one study, researchers found that CB1 receptors help <\/span><a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC4737832\/#:~:text=Our%20results%20indicate%20that%20CB1R%20agonists%20down%2Dregulate%20mast%20cell%20activation%20and%20may%20be%20used%20for%20relieving%20inflammatory%20symptoms%20mediated%20by%20mast%20cell%20activation%2C%20such%20as%20atopic%20dermatitis%2C%20psoriasis%2C%20and%20contact%20dermatitis.\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">down-regulate mast cell activation<\/span><\/a><span style=\"font-weight: 400;\">. This means CB1 activators like THC could help to reduce the inflammatory response brought on by mast cell activation, particularly with psoriasis and some forms of dermatitis.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Likewise, cannabidiol (CBD) may play an important role in MCAS relief. We have evidence that CBD may induce <\/span><a href=\"https:\/\/academic.oup.com\/jimmunol\/article-abstract\/194\/11\/5211\/7973167?login=false#:~:text=Together%2C%20the%20results%20suggest%20that%20CBD%20may%20induce%20activation%20of%20PPAR%2D%CE%B3%20in%20mast%20cells%20leading%20to%20secretion%20of%20G%2DCSF%20and%20consequent%20MDSC%20mobilization.%20CBD%20being%20a%20major%20component%20of%20Cannabis%2C%20our%20study%20indicates%20that%20marijuana%20may%20modulate%20or%20dysregulate%20the%20immune%20system%20by%20mobilizing%20MDSC.\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">activation of receptor protein PPARy<\/span><\/a><span style=\"font-weight: 400;\">, which can suppress immune system reactions. Additionally, because cannabidiol can inhibit certain enzymatic activity, research indicates that CBD may <\/span><a href=\"https:\/\/www.mdpi.com\/2227-9059\/11\/2\/520#:~:text=As%20cannabidiol%20(CBD)%20is%20known%20to%20inhibit%20IDO1%E2%80%B2s%20enzymatic%20activity%2C%20we%20hypothesized%20that%20pharmaceutical%2Dgrade%20CBD%20is%20an%20effective%20treatment%20for%20mastocytosis%2Dassociated%20pain\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">effectively treat mastocytosis-related pain<\/span><\/a><span style=\"font-weight: 400;\">.\u00a0<\/span><\/p>\n<h2><b>Anecdotal Evidence of Cannabis for MCAS<\/b><\/h2>\n<p><span style=\"font-weight: 400;\">While we have some supportive scientific evidence revealing that cannabis may help with MCAS, we have even more anecdotal, real-world evidence. We hope to see more scientific studies emerge that can help us make sense of the anecdotal cases.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Ticked Off Mast Cells, a resource group for people with mast cell and tick-borne diseases, conducted a survey to get a better idea of how medical cannabis can help with MCAS. They <\/span><a href=\"https:\/\/tickedoffmastcells.org\/?p=1582#:~:text=A%20total%20of%20114%20diagnosed%20or%20suspected%20mast%20cell%20activation%20syndrome%20patients%20took%20part%20in%20this%20survey.%2072%25%20of%20patients%20were%20diagnosed%20with%20MCAS%20and%2028%25%20of%20respondents%20reported%20suspected%20MCAS.%20Patients%20ranged%20from%2018%20to%2075%20years%20of%20age.\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">surveyed 114 people<\/span><\/a><span style=\"font-weight: 400;\"> diagnosed with (72%) or suspected to have (28%) MCAS. Respondents ranged from 18 to 75 years of age, with the majority (91%) identifying as female.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Survey results indicated that 85% of respondents found improvement when treating their MCAS with tetrahydrocannabinol (THC). This was a significantly higher success rate than any other treatment listed in the survey.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Of the many symptoms respondents reported on, 46% stated that cannabis helped the most with skin itching, swelling, hives, and rashes. As for <\/span><a href=\"https:\/\/www.veriheal.com\/blog\/cannabis-for-gut-health-ecs-microbiome-digestion\/#:~:text=Cannabis%20may%20relieve%20common%20symptoms%20of%20GI%20disorders%2C%20including%20abdominal%20pain%2C%20nausea%2C%20vomiting%2C%20and%20diarrhea.\"><span style=\"font-weight: 400;\">abdominal symptoms<\/span><\/a><span style=\"font-weight: 400;\"> like cramping, diarrhea, nausea, and vomiting, 77% of respondents found symptom relief with cannabis. The survey results showed respondents found relief from several other symptoms, including nervous system issues, pelvic pain disorders, asthma-like symptoms, and many others.\u00a0<\/span><\/p>\n<h2><b>Final Thoughts on Cannabis for MCAS<\/b><\/h2>\n<p><span style=\"font-weight: 400;\">While we have a decent amount of scientific and anecdotal evidence suggesting that cannabis may help with symptom relief for MCAS, know that<\/span><b> none of the information in this article should be taken as medical advice<\/b><span style=\"font-weight: 400;\">. If you are wondering about using cannabis for MCAS, be sure to speak with your healthcare provider to see if it may be a good fit.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">If you decide to try it, be prepared for some trial and error. Across MCAS spaces on the internet, some patients mention that certain consumption methods can <\/span><a href=\"https:\/\/www.reddit.com\/r\/MCAS\/comments\/13y4x7l\/whats_yalls_experience_with_mcas_and_marijuana\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">actually cause person-specific reactions.<\/span><\/a><span style=\"font-weight: 400;\"> However, everyone\u2019s immune system is different and some are hypersensitive or even paradoxical \u2013 we simply do not have enough understanding at this time to know why few react negatively. Regardless, you may find that certain methods work better for symptom control than others.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Respondents of the aforementioned survey noted the most <\/span><a href=\"https:\/\/tickedoffmastcells.org\/?p=1582#:~:text=64%25%20saw,with%20Sativa%20strains\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">symptom relief from indica <\/span><\/a><span style=\"font-weight: 400;\">strains, at 64%. An additional 45% reported success with hybrid strains, and 29% reported success with sativa strains. This may be because <\/span><a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC7763918\/#:~:text=%CE%B2%2Dmyrcene%20is,aromatherapy%20%5B66%5D.\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">indica strains carry particular terpenes<\/span><\/a><span style=\"font-weight: 400;\"> that cause their characteristic effects; it may be smart to look for similar terpene profiles in cannabis products \u2013 but there is no clinical research on this yet.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">We look forward to seeing more published scientific studies examining the relationship between cannabis and MCAS.
