After the appointment is complete, the doctor will fill out a recommendation form for medical marijuana and approve you. Once you have this you can then use that to apply to the state.
Once approved, the doctor will mail you a Practitioner Form, aka your medical marijuana recommendation. You need to forward this form to the RI state office along with the state fee and required documents listed on the state application.
The state will get back to you within 14 days to come in and take a photo for your medical cannabis card. You get the card right then and there. Once your card is in hand, you can visit a dispensary and begin purchasing marijuana legally.

A registered caregiver must be at least 21 years old to participate in the program. Registered caregivers will be required to provide proof of residency and complete a National Criminal Information Center (NCIC) background check from a local police department, the Rhode Island State Police, or with the Attorney General’s Office.
Qualifying patients may designate one authorized purchaser to assist in purchasing and delivering marijuana from a compassion center or cooperative cultivator. An authorized purchaser must be at least 21 years old and must complete an NCIC background check as well. An authorized purchaser may live out of state.
There is currently no online registry. Patients may complete an application online, but all applications must be mailed to the DOH for review and approval. The application process takes 2-4 weeks from the date it is received by the DOH. Patient and caregiver registrations issued or renewed after Jan. 1, 2017, will expire one year from the date issued. Renewal applications are sent out 60 days before your expiration date.
Adult patients and patients younger than 18 must meet certain requirements for DOH approval. For minors, the patient’s parent or legal guardian must be designated as their caregiver and/or authorized purchaser.
Residents of Rhode Island who have been diagnosed with a qualifying debilitating medical condition have the right to obtain and use medical cannabis if they have received a physician’s written certification form.
A patient cardholder who has in his or her possession a registry identification card shall not be subject to arrest, prosecution, or penalty for the medical use of marijuana, provided that the patient cardholder possesses an amount of marijuana that does not exceed twelve (12) mature marijuana plants and two and one-half (2.5) ounces of usable marijuana. Said plants shall be stored in an indoor facility.
A primary caregiver cardholder, who has in his or her possession, a registry identification card, shall not be subject to arrest, prosecution, or penalty for assisting a patient cardholder, to whom he or she is connected through the department’s registration process, with the medical use of marijuana; provided that the primary caregiver cardholder possesses an amount of marijuana that does not exceed twelve (12) mature marijuana plants and two and one-half (2.5) ounces of usable marijuana for each patient cardholder to whom he or she is connected through the department’s registration process.
A cardholder shall be allowed to possess a reasonable amount of unusable marijuana, including up to twelve (12) seedlings, that shall not be counted towards the limits of this section.
